We try to simplify the preparation of your insurance claims. It is your responsibility to furnish us with the necessary information regarding your insurance so that a claim may be filed for you.
To assist you in understanding our office policy regarding insurance, we wish to bring the following to your attention: Dental and Medical insurance are not designed to be a "Pay-All" ; it is intended to be an aid in attaining optimum health.
Our fees for services are the same for all patients, whether or not they have an insurance program. Your particular insurance may base its allowances on a fee schedule which may or may not coincide with our office fees. Should there be a differenence in the reimbursement you receive from your insurance and our fees, this is understandably a matter between you and your insurance company.
Our professional services are rendered to you, not the insurance company. We do not have a contract or agreement with insurance companies to accept payment determined by them as payment in full for services rendered to their policy holders. Therefore, you are directly responsible to us for the payment of all services rendered to you.
